There are new guidelines for admitting terminally ill clients to nursing facilities.

Please gather:

• Physician's Statement that client is terminally ill with a life   expectancy of 6 months or less if the terminal illness runs its normal course.
• Name of Hospice
• Name of Hospice Contact Person
• Hospice Fax Number
• Name of Nursing Facility
• Address of Nursing Facility

It is no longer necessary for the PASRR (columns A&B) portion to be completed for residents how are Hospice clients.

This new policy only applies to terminally ill clients with a physician's statement.

Fax the aforementioned items to the Area Agency on Aging at 785-628-2005.  The client will receive a letter from KDOA indicating that they were exempt from CARE.  This letter takes the place of the traditional CARE assessment and certificate.

Please follow the same procedure if the client has received the physician's statement but has not requested the assistance of Hospice.
