Spring 2009 Reminders...

  • It's a lot easier on the client and their family to have us fax you the CARE paperwork than to have the client/family track it down!  Just give us a call at 1-800-432-7422 and we'll fax the documents to you.

  • Be sure to give the client / family all available area nursing facility options.  If there is a waiting list at the client / family's desired location, please offer to get the client placed on a waiting list.

  • If only a PASRR is completed no CARE Certificate is issued

  • CARE / PASRR is not required for the Soldier's Home in Fort Dodge.

  • When a client seems quite independent please call the Area Agency on Aging.  If the assessment has a particularly low score, I will be calling you to make sure the assessment accurately reflects the client's abilities.

  • Assisted Living is NOT living alone.

  • Orientation section - must be coded with only 1, 0 or 9.

  • Please alert the CARE program of name or employment changes so we can update our records.

  • A Power of Attorney is NOT the same as a Guardian.

  • If Guardianship is involved, please indicate where guardianship papers can be found or include a copy with the assessment.

  • Please document EVERYTHING!  Don't expect me to know anything about a client - explain everything and document situations thoroughly.

  • Clients with cognitive impairments typically have impaired decision making skills and need supervision on ADLs and IADLs.  Please make sure your coding reflects this.

  • If a client has frequent falls, hands-on assistance (3) is needed in bathing, dressing, and housecleaning.  In the comment section, please document where falls happened.

  • Fax full CARE assessments to the Hays office, regardless of where the client's residential address is if your workplace is within our 18 county area.  The nursing facility expects a fax of the CARE certificate and the client retains the goldenrod copies of the entire assessment.

  • It is recommended to have at least 2 trained staff members to complete CARE / PASRR.  These individuals may be Administrators, Directors of Nursing, RNs, or Social Workers.

  • If you suspect you may have had fax issues, please call to verify the AAA received it so we can prevent payment issues.

  • When a Nursing Facility admits a new client it is beneficial to check that the AAA has received the CARE assessment from the assessor to prevent payment issues.

  • All clients who attempt the clock draw should be told to place the hands on the clock at 10 minutes after 11 o'clock.
